Hemorrhagic Pneumonia
The gross pathology specimen shows a hemorrhagic lobar pneumonia (red hepatisation) in the left upper lobe with patchy subsegmental bronchopneumonic disease in the lower lobe. The relationship to the surface pleura may result in pleuritic pain.

Lobar Pneumonia with a Filling Defect in the Pulmonary Vein
Left Lobar Pneumonia, Left Pleural Effusion, with Decreased Ventilation and Perfusion of the Diseased Left Lobe The upper panels are imaged during early pulmonary venous phase and show a “filling defect” in the left lower lobe pulmonary vein in the axial and coronal reconstruction of a CT scan. The lower panels are in equilibrium phase and show normal opacification. The “filling defect” in the upper panels is caused by a delay in opacification (ie blood flow) to the left lung because the poor ventilation in the left lung has resulted in a decreased perfusion and therefore there is unopacified blood in the left lower lobe pulmonary veins. The right lung is normal and pulmonary veins are opacified normally. The lower panels are in the later equilibrium phase of contrast opacification, and by this time they have become opacified with contrast confirming the absence of a suspected thrombus in the left lower lobe pulmonary vein.
